Interactive screening tools
The BMI and foot screening tools also request parent contact details and send the entered information and screening summary through the website’s contact form when the visitor consents and requests a result or consultation. Inquiries from these forms are directed to Transform Kid’s existing consultation mailbox, transformgym.vn@gmail.com.
The original height, BMI and foot screening tools use information entered by the visitor to generate a preliminary result. The height tool includes a step asking for a parent’s contact number to unlock the result. Continuing at that step sends the contact and child details shown in the tool, including age, sex and height, to Transform Kid’s existing inquiry system hosted through Google Apps Script. Further registration can include the selected location and inquiry goal. The team may use these details to follow up about the program.
The height tool may retain campaign parameters in browser session storage and include them with an inquiry. Please use the tools only as a parent or authorized caregiver.
